If you are self referring you will need to fill out the "Pre intake questionnaire"
if you are being referred from a clinic or another organization the provider will need to fill out a "Client Referral Form" under the "Intakes and Referrals" Tab.
"If you don’t hear back within three of our business days (Tuesday through Thursday), please feel free to follow up to confirm your appointment and ensure your forms were received."
Effective February 24th: the Lotus Centre is currently booking 4 weeks ahead for Support Worker appointments.
When we reach out to schedule a Support Worker appointment we can discuss other resources that might be helpful as well.
