If you are self referring you will need to fill out the "Pre intake questionnaire" (below)
if you are being referred from a clinic or another organization the provider will need to fill out a "Client Referral Form" under the "Intakes and Referrals" Tab.
"If you don’t hear back within three of our business days (Tuesday through Thursday), please feel free to follow up to confirm your appointment and ensure your forms were received."
Effective July 2026: the Lotus Centre is currently booking 2-4 weeks ahead for Support Worker appointments.
